Speech Language Pathologist (SLP) Flagstaff AZ

Schedule : MondayFriday 8 : 00 AM 5 : 00 PM Pay : $75 / hour No weekends or overtime

Position Summary :

We are seeking a licensed Speech Language Pathologist to evaluate and treat residents / patients with speech hearing swallowing and cognitive challenges. This role involves providing therapy developing treatment plans and collaborating with physicians rehab staff and nursing teams. Entry-level candidates are welcome to apply.

Requirements :

Bachelors degree in Speech Language Pathology

Active Arizona SLP license

TB test (2nd Step or QuantiFERON Gold)

CPR / BLS Certification

Key Responsibilities :

Assess patients and create individualized therapy plans

Provide direct therapy and monitor progress

Educate residents / patients and families on therapy techniques

Maintain accurate documentation and comply with HIPAA

Collaborate with interdisciplinary teams

Compliance Items Needed :

Resume Level 1 Fingerprint Clearance OIG / SAM Check CPR / BLS Card TB Test License / Certification Hepatitis B & COVID vaccination or declinations Agency Skills Assessment Packet
