Key Responsibilities :

• Assist in the day-to-day provision of active support and programming tailored to each participant’s support plan
• Promote and protect the civil and personal rights of program participants
• Help maintain the physical and emotional well-being of clients in accordance with their individualized program plans
• Support participants in daily activities, including personal care, household tasks, and community engagement
• Document and report any changes in participants’ condition or behavior to appropriate team members
• Foster a respectful, positive, and inclusive environment for all participants
• Collaborate with team members to develop and implement effective support strategies
