Travel Licensed Practical Nurse (LPN) – SNF

Shift : Nights | 7 : 00 PM – 7 : 30 AM (flexible to 11 : 00 PM – 7 : 00 AM)

Weekend / Holiday Requirements : Every other weekend and holidays required

Position Summary :

A facility in Oneida, NY is seeking a travel LPN to provide high-quality care to residents at their Rehabilitation and Extended Care Facility . The LPN will collaborate with the Unit Manager and work under the supervision of the RN and physician, overseeing CNAs and Ward Clerks while ensuring residents’ daily care needs are met.

Responsibilities :

Provide direct patient care to residents, including medication administration, vital signs, and monitoring overall health

Supervise CNAs and Ward Clerks on the unit

Communicate effectively with RNs, physicians, residents, and families

Exercise independent judgment within scope of practice while maintaining collaboration with the healthcare team

Document care and patient progress in accordance with facility and regulatory requirements

Requirements :

Active New York State LPN License – Required

Minimum 2 years of LPN experience – Required

BLS Certification – Required

Prior SNF or rehab experience – Preferred

Strong communication skills and professional work ethic

Schedule :

Nights | 7 : 00 PM – 7 : 30 AM (flexible to 11 : 00 PM – 7 : 00 AM)

Every other weekend and holidays required

13-week assignment
