Job Description
We are seeking a compassionate, autonomous, and experienced Nurse Practitioner (NP) to join our growing healthcare team. In this role, you will provide high-quality, personalized, patient-centered primary care to individuals in their homes and through telehealth visits. Approximately 10% of your time will be dedicated to in-person house calls, with the remainder conducted virtually. Unlike traditional clinic settings that prioritize high patient volume, we focus on giving each patient the time and attention they deserve. Key Responsibilities :
Responsibilities are structured to allow you to spend the necessary time with each patient, without pressure to rush visits or meet unrealistic volume targets.
- Conduct comprehensive primary care assessments via telehealth and in-home visits , including health history, physical exams, and diagnostic testing when necessary
- Diagnose and manage acute and chronic primary care conditions in both virtual and in-person settings
- Develop, implement, and manage personalized care plans tailored to patient needs and preferences
- Prescribe and manage medications in accordance with state licensure and scope of practice
- Provide health education and counseling to patients and caregivers on preventive care, disease management, and lifestyle modification
- Coordinate care with patients’ primary care physicians, specialists, and other healthcare providers to ensure continuity of care
- Document all patient encounters in the electronic health record (EHR) accurately and in a timely manner
- Ensure compliance with HIPAA and other applicable regulatory standards
- Manage and respond to urgent and non-urgent patient needs during both telehealth and in-home visits
Qualifications :
Active Family Nurse Practitioner license with prescriptive authority in UtahMust be able to see patients of all agesMinimum of 2 years of experience in primary care, family practice, or internal medicineStrong clinical skills in diagnosing and managing a variety of chronic and acute conditionsProficiency with electronic health records (EHR) and mobile health technologyAbility to work independently with a focus on quality over quantity of patient interactionsValid driver’s license and reliable transportation to travel to patient homesCurrent CPR / BLS certificationStrong interpersonal and communication skills, with a focus on patient education and collaborationPreferred Qualifications :
