Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, assisted living facilities, and adult day care environments. As an LPN, you will work under the supervision of the Executive Director.

The LPN currently serves as the nurse of record for the facility and subsequently must be licensed under the Oklahoma Board of Nursing with a minimum credential of Licensed Practical Nurse. The LPN is responsible for renewing license as indicated and must hold credentials with no encumbrances or disciplinary actions.

Responsibilities

• Responsible for the collection of information from the inquiring family member / care giver / participant to determine their appropriateness for Full Circle’s service. The Nurse is responsible for obtaining their enrollment in the event the participant is determined to be appropriate for Full Circle’s service.
• Responsible for acquiring and maintaining all documentation required by the State Department of Health to include but is not limited to : DNR, Medical Assessment, Medication Administration Sheet, Meal Percentage Sheet and Dietary needs, Vital Sign / Weight Tracking, Care Plans, Monthly Summaries, and Participant’s Check-ups by their physician.
• Provide direct supervision to CNA's and ensures all staff certifications are updated in a timely manner, conduct annual reviews and employee discipline of direct reports in concert with Executive Director.
• The LPN will oversee all functions of Kitchen operation including the assembly of the monthly menu with consult from the dietician, ordering of food and other center supplies, and oversight of the required documentation for the food program.
• The LPN / LPN is responsible for communicating information and concerns between physicians and family members.
• The LPN will coordinate and / or provide in-service trainings for the program staff one time a month.
• The LPN is the full-time licensed nurse on staff and administers and / or oversees CMAs / MATs administration of medication.
• The LPN starts and completes an individualized care plan on each participant within 10 days of admission, and updates when changes or every six months.
• The LPN maintains monthly progress reports.
• Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
• Perform wound care and other nursing procedures as required.
• Maintain accurate medical records and documentation in compliance with HIPAA regulations.
• Collaborate with healthcare team members to coordinate patient care.
• Support patients with activities of daily living (ADLs) such as bathing, dressing, and mobility assistance.
• Participate in infection control measures and maintain a clean and safe environment for patients.
