What You’ll Do

As a Paneled Provider , you’ll manage your own group of patients and provide personalized, relationship-based care right in their homes. You’ll partner with an interdisciplinary team to keep patients healthy, independent, and connected to community resources.

Key Responsibilities :

• Conduct comprehensive assessments and develop individualized care plans.
• Manage chronic conditions and respond to urgent needs.
• Provide preventive and primary care visits in-home.
• Collaborate with physicians, nurses, and care coordinators.
• Ensure all care follows evidence-based standards and compliance requirements.
• Educate and empower patients to take control of their health.

What You’ll Bring

Active Nurse Practitioner (NP) license in Florida .

2+ years of clinical experience in primary care, geriatrics, or home health.

Certification from AANP or ANCC (or equivalent).

Strong communication skills and compassion for older adults.

Commitment to excellence, empathy, and continuous learning.
