Job Description

We are seeking a dedicated and experienced Care Manager (LCSW or RN) to support hospital care management and discharge planning. This role is critical in coordinating patient care, facilitating safe discharges, and ensuring continuity of services across care settings.

MUST HAVE

• Active LCSW license or RN license.
• 2+ years of hospital case management and discharge planning experience.
• Proficiency in Epic charting system.
• Strong knowledge of discharge planning, care coordination, and community-based services.
• Previous charge experience preferred.
• Background in teaching or community hospitals, trauma centers preferred.
• Experience with protective services, hospice, and long-term care coordination preferred

Responsibilities

Work Environment

This position is located at a Short Term Acute Care / Level IV Trauma Center with 60 staffed beds and a daily census of approximately 40 patients. The schedule is Monday to Friday, 9 : 00 AM – 5 : 30 PM, with a requirement of 2 weekend shifts every pay period and potential floating to other locations. Orientation is 8 hours. The contract duration is 18 weeks, with potential extensions, and the opportunity to go direct based on performance and attendance. Scrub color is any color or business casual, and free parking is available.

Pay and Benefits

The pay range for this position is $40.00 - $55.00 / hr.
