Part-Time Social Worker

Salmon Brook Rehab and Nursing is a short and long term care facility located in Glastonbury, CT. We are currently seeking looking for a compassionate, motivated Part-Time Social Worker to join our team. If you’re all about supporting residents, advocating for their needs, and helping families navigate care with confidence, this role is a great match.

What You’ll Do :

Provide emotional support and guidance to residents and families

Assist with care planning, assessments, and discharge coordination

Work with the interdisciplinary team to ensure residents’ needs are fully met

Help residents access resources and community services

Maintain accurate documentation and support smooth communication across departments

What You Bring :

Valid CT Social Work license (or eligibility)

Strong communication + problem-solving skills

Empathy, patience, and solid follow-through

Experience in a long-term care or healthcare setting is a major plus
