Care Gap Project Lead
Serves as the lead for the coordination and project management of Care Gap projects that utilize operational methodologies to drive substantial improvements in quality, efficiency and patient engagement. Provides guidance and direction to the Keystone ACO Partner Organization Clinical Practice Transformation team who will assist with the implementation of action plans developed to improve identified Care Gaps and Quality measure. Provide education and supportive data concerning quality measure benchmarks, status and improvement needs for primary and specialty care providers in multiple practice sites to facilitate clinical guideline adherence and coordinate services specifically related to the patient's health care needs.
