Overview

Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury. Provides age-appropriate education for clients and caregivers. Supervise and assume responsibility for care provided by identified team members.

Education

  • Bachelors Physical Therapy (Required)
  • Masters in Physical Therapy Degree (Preferred)
  • Doctorate in Physical Therapy

Certification / Licensure

  • Physical Therapy License (Required)

    • Experience

  • Performs modalities / therapeutic interventions in accordance with accepted standards of care and department policies and procedures.
  • Provides effective instruction to the patient and family through standardized educational programs and / or individualized training. Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes.
  • Productive and efficient in daily operations.
  • Selects and performs the appropriate assessment procedures.
  • Establishes and updates formal plan of care to address individual patient needs.
