An opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health to provide skilled nursing care in patient homes within the Commerce, GA area. This full-time contract position offers a structured Monday through Friday schedule, from 8 : 00 AM to 5 : 00 PM, with a caseload averaging 2532 points weekly (typically 35 patient visits per day). The role requires daily travel within an assigned territory, with mileage reimbursement provided.

Responsibilities :

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify condition changes, and communicate promptly with care teams and physicians.
• Educate patients and families regarding disease processes, treatments, medications, and preventive care.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary teams including physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with regulatory and agency standards.
