Director of Compassionate Care

A company is looking for a Director, Compassionate Care Program.

Key Responsibilities

Oversee proactive, high-quality, patient-centered care and advance care planning

Develop and implement workflows to engage patients and providers in palliative and end-of-life care

Monitor performance metrics and collaborate with care staff to ensure effective patient care transitions

Required Qualifications

Bachelor's degree and at least 7 years of healthcare experience as a Registered Nurse, Nurse Practitioner, or Physician Assistant

Minimum of 4 years of leadership experience in palliative care, hospice, or end-of-life care

Proven ability to lead teams and drive performance improvements in clinical quality

Experience in conducting goals of care discussions with patients and families

Intermediate skills with MS Office Suite and ability to work effectively in a remote environment
