A company is looking for a Senior Director, Clinical Excellence (Post Acute, VBC).

Key Responsibilities

Oversee the delivery of the Post Acute Care Program ensuring seamless patient transitions and improved outcomes

Drive optimal performance of the PAC clinical team through engagement and development

Collaborate with stakeholders to ensure high-quality PAC care delivery and compliance with regulations

Required Qualifications

Unrestricted RN, NP, or PA state license

10+ years of clinical leadership experience

Master's degree or higher in a related field preferred

Experience in value-based care models and establishing KPIs

Proven track record in improving clinical quality and outcomes
