• Free ParkingDirector of Social Services DutiesPlan, organize, and direct all functions of the Social Services DepartmentComplete and oversee social service assessments, care planning, and documentationSupport residents and families with discharge planning, care transitions, and access to financial, legal, or community resourcesFacilitate grievance resolution and advocate for residents' rights and dignityParticipate in interdisciplinary care team meetings and QAPI initiativesOversee advance care planning and end-of-life discussions in accordance with resident wishesSupervise and mentor social work staff, ensuring compliance with federal and state regulationsParticipate in resident and family councils, community outreach, and facility committeesUphold confidentiality, ethical practice, and professional standards of careRequirementsBachelor's degree in Social Work or a related human services field (sociology, psychology, gerontology, etc.)At least one year of supervised social work experience in a healthcare settingKnowledge of state and federal long-term care regulationsStrong leadership, communication, and problem-solving skillsCompassionate, organized, and team-oriented approachOur Caring CommunityMystic Healthcare is a family-owned skilled nursing facility specializing in short-term rehabilitation and long-term care.
