Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 though community education, emergency services, and outpatient care. As we continue to expand our services & locations, our community has grown far beyond Paris. Position Summary :

In accordance with department policy and the patient’s plan of care, the RN provides direct and / or indirect nursing care, treatment, and instruction in the homes of patients referred by the Home Care department.

Essential Functions :

  • Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various complex home situations
  • Participates in the on-call system as directed, providing availability of skilled nursing services at all times according to agency policy
  • Teaches and supervises family members, auxiliary nursing personnel, or others giving nursing care
  • Coordinates with all services in the home
  • Plans with the family and physician for services within the physical, financial, and emotional resources of the family; helps the family accept responsibility of providing care
  • Gives prescribed treatments or emergency care, teaches, and supervises patients, family members or others who give such treatments
  • Initiates nursing measures to prevent complications or to minimize the disabling effects of disease or injury
  • Observes, evaluates, and reports to physician patient’s physical and emotional condition, reaction to drugs or treatments, and changes in condition or needs
  • Performs diagnostic tests as authorized and obtains laboratory specimens when indicated
  • Completes accurate documentation in medical record of services provided in accordance with the plan of care
  • Helps individuals and families to accept and adjust positively to physical, mental, and social limitations
  • Complies with policies and procedures of the organization / department
  • Participates in quality improvement activities
  • Makes necessary revisions in the Plan of Treatment
  • Participate in in-service programs for agency staff
  • Provides comprehensive assessment, planning, implementation, and evaluation for home health patients as the primary nurse
  • Assesses physical, functional, psychosocial, and cognitive status of the home care patient
  • Assesses the home environment for safety, infection prevention, and community resource needs
  • Collects information from the patient, family, physician, and other community resources
  • Incorporate multidisciplinary data into the nursing assessment of the home care patient
  • Assess the presence of advanced directives and facilitate further action in obtaining or implementing if indicated
  • Plans with the patient, family, and physician for care which is feasible within the physical, financial, and emotional resources of the family
  • Establish individualized, measurable goals in consultation with patient, family, and other health care providers
  • Maintain a working knowledge of community resources and refer patients appropriately
  • Utilize home care standards, regulatory guidelines, and standards of care in developing an individualized care plan
  • Maintains technical skills according to organization standards as measured by competency assessment during orientation and annually / as needed
  • Implements safe, competent care with home care patients and families with organization policies, procedures, and standards of care
  • Accurately follows established infection prevention and safety policies and procedures
  • Directs the use of equipment and supplies in an efficient and cost-effective manner
  • Coordinates and directs home care aide assignments, prepare written instructions, and supervises the aide in the home
  • Coordinates and directs Licensed Practical Nurse activities, prepares care plan, and provides oversight and supervision of care and documentation
  • Sets priorities of home care patients adapting to changing needs of patients and families
  • Administers medication and treatment as prescribed by the patient’s physician, podiatrist, or advanced practice nurse or physician assistant
  • Documents components of the nursing process to reflect integrated approach to care
  • Maintains a clinical record for each patient receiving care
  • Provides a progress note to the physician when there are deviations in care or at least every 60 days
  • In response to information indicating a change, need for urgent care, or potential emergent patient risk, notifies the physician immediately and the supervisor of the action taken
  • Serves as role model and resource to other nurses regarding delivery of care to patients, and families
  • Evaluates patient’s responses to care based on a continuing assessment and analysis of nursing intervention and alternatives for nursing care and initiates needed changes
  • Informs physician, nurse manager, and other appropriate members of the health care team of changes in the patient’s condition and needs
  • Facilitates and coordinates interdisciplinary care conferences with groups of complex patients
  • Exhibits sound nursing judgement and decision-making skills in coordinating patient care
  • Identifies and adapts teaching materials with patients and their families- creatively using these tools to maximize the learner’s success
  • Identifies individuals’ requirements / perceptions of learning needs
  • Continually assesses patient / family level of understanding and adjusts teaching and plan of care accordingly
  • Documents patient, family, and caregiver teaching and patient / family response
  • Maintains a complete record of care that is provided to the patient per home care policy and procedure
  • Meets required deadlines for documentation for OASIS, certification, recertification, aide supervision reports, aide care updates, licensed practical nurse oversight, case coordination, care plan updates, verbal orders, and progress towards goals
  • Accurately completes admission and revisit documentation, completed communications, billing, and employee statistical information, and payroll documentation
  • Documents all communication with payers, external providers, and providers
  • Assist in creating a positive climate conducive to learning
  • Acts as a resource person on clinical problems for other personnel caring for home care patients
  • Acts as a role model for professional nursing and allied health students accompanying them on home visits as assigned
  • Fosters the professional development of self, clinicians, and other home care staff
  • Evaluates performance of peers as appropriate in relation to home care standards of practice
  • Serves as preceptor for new home care employees
  • Participates in committees, education programs, staff meetings, nurse meetings, in-service programs, staff development, and mandatory education requirements for licensure
  • Demonstrates flexibility in adjusting availability throughout the workday and responds to office within expected time frames
  • Treats all people with respect and dignity
  • Understands own behavior and how it affects others
  • Notifies patients of expected time for the day’s visit by 8 AM, allowing 15 minutes before / after expected arrival time
  • Understands and attempts to meet the needs, wants, and expectations of internal and external customers
  • Addresses concerns or complaints by patients, families, caregivers, or other internal and external customers and notifies manager of these concerns
  • Displays professional attitudes and behaviors of trust, respect, honesty, and caring
  • Enables co-workers, patients, and families to verbalize their need for assistance
  • Demonstrates understanding of differences in cultural, spiritual, and socioeconomic backgrounds
  • Identify ethical conflicts and notify managers, and participate in problem solving with involvement of manager, primary physician, and social services or ethics group
  • Maintains patient privacy and confidentiality
  • All duties and requirements must be performed consistent with Home Care policies
  • Promotes the mission and values of the organization
  • Ability to work independently
  • Ability to handle difficult situations involving patients, providers or others in a professional manner

The responsibilities listed above are not all-inclusive; other activities may be required in support of the hospital's goals and objectives.

Position Requirements :

  • Registered Nurse with current license in the State of Illinois
  • Minimum of one (1) year of relevant RN experience, preferred two (2) year acute / LTC experience
  • Reliable transportation with current Illinois driver’s license and proof of auto insurance
  • Strong leadership, decision-making, problem solving and organizational skills
  • Must be willing to participate in quality improvement process
  • Graduated from an accredited school of nursing
  • Current license as RN in State of Illinois
  • Current CPR certification or within thirty (30) days of hire
  • Ability to communicate well verbally and in writing
  • Knowledge of nursing process and use of the same
  • Ability to perform complex and varied tasks in a fast-paced work environment subject to stress and interruption
  • Position Information :

    Location : 798 E. Court Street Paris, IL 61944

    Hours : Monday- Friday dayshift with some on call hours (1 week every 4-6 weeks typically)

    Pay Range :

    Pay ranges from $26.688 / hour to $42.740 / hour (rate of pay is based on applicable years of experience)
