We are looking for Medical Assistants who are inspired to deliver a great experience to our patients and who want to play an important role in patient-centered care by performing at the top of their scope of practice. At La Clnica, you will be challenged to be exceptionally accurate, observant and an efficient taskmaster while at the same time providing a human touch and excellent overall patient experience. You should have a successful history of being responsible, reliable, and capable of solving problems while being friendly and helpful looking for ways to support other care team members.

Major areas of responsibility include but are not limited to :

• Work directly with Integrated Behavioral Health Clinician (IBHC), Case Managers (CM), and Registered Dietitians (RD) to ensure optimal scheduling and triage of patients who are referred for IBH / CM / RD services over the phone and in person.
• Make reminder calls and follow up on patients with missed visits to reschedule patients.
• Process and track both internal and external referrals.
• Create and manage IBHC(s), CM(s), and RD(s) schedules and templates.
• Provide administrative assistance to IBHCs, CMs, and RDs running groups by greeting patients and administering pre-group screens and measures.
• Conduct vitals and point of care testing such as A1c's in group visits.
• Administer and ensure completion of objective health measures and pre-visit screenings (PHQ-9, ACES)
• Liaise and ensure coordination between Medical & IBH staff (in-basket messaging, Epic / Teams chats, and / or other tasks assigned.
• Alert clinicians to time sensitive clinical needs.
• Assists with and coordinates warm handoffs.
• Coordinate interpreter as needed.
