Title : Licensed Practical Nurse (LPN / LVN)

Duration : 9 Weeks

Location : 262 Toll Gate Road, Langhorne, PA

Schedule : 7 : 00 AM – 3 : 30 PM | 30-Minute Meal Break

Position Summary

The Licensed Practical Nurse (LPN / LVN) provides safe, efficient, and effective nursing care under the supervision of a Registered Nurse (RN). This role focuses on achieving positive clinical outcomes, patient satisfaction, and compliance with all applicable nursing standards. The LPN contributes to patient assessments, implements individualized care plans, and oversees unlicensed personnel to ensure the highest quality of care.

Key Responsibilities

Collect, document, and report patient data; observe and communicate changes in condition to the RN or physician.

Assist in developing, implementing, and evaluating individualized care plans to achieve patient goals and minimize rehospitalizations.

Administer medications and perform treatments per physician orders, ensuring accurate and timely documentation.

Supervise and delegate tasks to unlicensed staff in accordance with the Nurse Practice Act and facility policies.

Participate in interdisciplinary care planning, discharge preparation, and patient / family education.

Maintain drug counts, assist with physician rounds, and ensure proper shift-to-shift communication.

Utilize PointClickCare (PCC) for accurate electronic documentation.

Uphold infection control, safety, and confidentiality standards, ensuring compliance with HIPAA and facility regulations.

Support orientation and training of new staff and participate in ongoing quality improvement and in-service programs.

Promote a respectful, team-oriented, and safety-focused care environment.

Qualifications

Graduate of an accredited school of practical or vocational nursing .

Active LPN / LVN license in the state of Pennsylvania.

Current BLS / CPR certification (AHA required).

Experience in rehabilitative or geriatric nursing strongly preferred.

Ability to read, write, and communicate effectively in English.

Verification of tuberculin skin test (TST) as required by state law.

Willingness to stay beyond scheduled shifts as needed to meet patient care and staffing needs.
