Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Minimum of 1 year of Home Health experience required.
• Experience with Homecare Homebase is required.

• Facility Type: Home Health
• Shift: Days
• Working Hours: 0800-2000
• Weekend Rotation: None
• No call or major holidays.
