JOB DESCRIPTION

Position : HCBA OCM Case Manager

Reporting To : HCBA Lead MSW HCBA Program Manager

Work Type : Remote

Department : HCBA 700

POSITION SUMMARY :

The HCBA Open Case Management (OCM) Case Manager is the case manager who oversees the social and emotional needs of the client and their families. The case manager ensures all necessary documentation and eligibility are met so that the client can have the support needed that is discussed in the Plan of Treatment (POT).

QUALIFICATIONS :

1. Masters of Social Work preferred BSW or Bachelors in a related field required.

2. Experience in a health care setting preferred.

3. Active drivers license.

4. Excellent verbal and written communication skills.

5. Proficiency in the use of computers.

6. Detail oriented and organized.

7. Proven ability to work in a faced paced environment.

8. Ability to meet assigned deadlines.
