Job Description :

• Supervise and coordinate clinical rehabilitation activities for home health patients.
• Provide direct physical therapy services to patients in their homes.
• Collaborate with a multidisciplinary team to develop comprehensive care plans.
• Ensure compliance with regulatory requirements and best practices in home health.
• Conduct assessments and evaluations to establish individualized treatment plans.
• Provide guidance and support to therapy staff and ensure high-quality patient care.
• Participate in staff training and development programs.
• Maintain accurate documentation and records of patient care activities.
• Communicate effectively with patients, families, and healthcare professionals.
