About The Position

Job Title : Physical Therapist

Hours per Shift : 32 hours guaranteed

Experience : New graduates are welcome; SNF experience preferred

License : Valid Physical Therapy License required

Certifications : Current CPR certification preferred

Must-Have :

• Completed prescreen form
• Resume
• License verification

Description :

This position involves working two days at one facility and three days at another site.

Candidates must ensure that the prescreen form is filled out and attached upon submission.

Both the resume and license verification also need to be attached.

If these items are not completed and attached, the candidate will be declined.

A COVID test is required 72 hours prior to the start date, with results issued 48 hours before starting.

Candidates should review the credentialing checklist prior to submission.

The checklist includes verification of state license, resume, three references, county criminal history for the past seven years, federal criminal history for the past seven years, a multi-state criminal search for the past seven years, a nationwide sex offender search for the past seven years, HHS / OIG search, GSA search, and social security number trace.

Candidates must also provide a two-step PPD, Quantiferon, or proof of a positive PPD along with a chest X-ray.

Oregon state background check will be facility-run.
