Job descriptionScreens and assess nutritional status of patients / clients seen for Diabetes Education and collaborate with patient to develop and implement a plan and document all pertinent information into the patient's electronic medical record requiring basic computer systems knowledge. Develops and implements care plans, coordinating services with diabetes management team and ancillary services. Train / educate patient's on blood glucose monitoring including / not limited to include continuous glucose sensor (CGMS), insulin pumps and prescribed medication use including injectables. Represents the system and diabetes program as a diabetes education care resource person. Participates in ongoing clinic / program development to include policy / procedures and quality initiatives as they relate to diabetes care, clinical development and efficiency. Enters appropriate charges into EPIC with strong attention to detail and accuracy.
