Description : JOB SUMMARY
The Assistant Director of Nursing (ADON) is responsible for ensuring the delivery of exceptional resident care of Health & Rehabilitation Center residents. This role focuses on the oversight of the admission process, the accuracy and compliance of care plans, and overseeing the wound care and at-risk program. The manager reports to the Director of Nursing and collaborates closely with other nurse leaders and interdisciplinary teams to enhance resident outcomes and ensure a positive resident experience.
ESSENTIAL JOB FUNCTIONS
Clinical Leadership
- Work with Nursing staff, Admissions Coordinator and other interdisciplinary team members to supervise the admissions process of short-term skilled care and rehabilitation residents.
- Assign resident care responsibilities to nursing staff and ensures care plans are developed, followed, and revised as needed.
- Rounds daily, ensuring skilled and long-term care nursing services are performed in accordance with acceptable nursing standards and takes immediate and appropriate action to correct concerns.
- Tracks changes in resident condition and ensures adjustment to the plan of care, communicates concerns and updates to the DON and IDT team.
- Oversees the wound care program, in conjunction with the wound care nurse, ensuring accurate wound care assessments and documentation.
- Ensures the integrity of infection control policies under the direction of the Infection Prevention program.
- Serves as Director of Nursing in the event of a long-term absence of the Director of Nursing.
- Works as a staff nurse when necessary and assumes on-call responsibility as assigned.
Communication, Collaboration & Coordination
Collaborate with Admissions Coordinator and DON to review resident referrals medical needs. Ensure the availability of appropriate resources and equipment to meet such needs and lead efforts to secure resources and items through WLI and vendor resources.Ensure communication of and follow up on pharmacy recommendations and lab and diagnostic results with appropriate medical providers.Verifies residents’ appointments during stay have been scheduled and follow up recommendations have been communicated with the resident’s responsible party and Williamsburg Landing reception.May participate in weekly Medicare and monthly triple check meetings to discuss resident status, discharge progress, and documentation accuracy. In conjunction with social services, communicates projected discharge timeline to resident and responsible parties.Collaborates with the Social Services / Discharge Planner to ensure resident readiness for discharge including resident education and medication reconciliationManages skilled resident and / or family concerns and reports to the Director of Nursing and Social Services / Discharge Planner for follow up.Compliance & Quality Assurance
Ensure all clinical requirements and the admission checklist have been completed by assigned staff per regulations and Williamsburg Landing job routines, policies and procedures.Reviews admissions documentation for accuracy and completeness to include medication reconciliation and verification of orders.Routinely monitors skilled nurses’ notes and aide’s ADL documentation to ensure accuracy and compliance with regulation.Assist the Director of Nursing in verifying competency with regard to medication pass, reviewing physician orders for accuracy and completionAttend clinical meetings, resident care plan meetings; Interdisciplinary Team Meetings, Risk Management, and QAPI Committee meetings; provides reports on assigned quality metrics.Conduct routine environmental rounds to ensure compliance with WLI, IP and CMS standards.Staff Management & Development
Assess the abilities of clinical staff, identifies areas of training, and coordinates with clinical educator and DON on training efforts.Responsible for the annual employee performance reviews and stay interviews for personnel assigned by DON or Administrator.Responsible with the DON for holding nursing ream accountable for essential responsibilities and issuing timely Corrective Action Notices (CANs) when necessaryCo-investigate with the DON and creates or executes action plans to respond to areas of opportunity identified through We Care Connect and Stay InterviewsCollaborate with the Scheduler to help fill open shifts by directly communicating with staff, encouraging flexibility, and ensuring adequate coverage.Actively support staffing needs during call-outs, high census, or emergency situations.Hold staff accountable for schedule adherence, including reliability, punctuality, and proper call-in / call-out procedures.Address attendance or tardiness issues promptly through coaching or corrective action in alignment with facility policies.Performs all other duties as assigned.QUALIFICATIONS
Current Commonwealth of Virginia Registered Nurse license, required.Two or more (2+) years of experience in skilled nursing or rehab facility, required.One or more (1+) years of experience in nursing supervision, preferred.Knowledge of state and federal Long-Term Care regulations, preferred.Experience with MDS tools, and assessment and planning processes, preferred.Knowledge, Skills and Abilities
Knowledge of geriatric and skilled nursing care standards, principles, and practices and of state and federal regulations related to nursing homesDemonstrated proficiency and experience in Electronic Medical Record (EMR) systems and other relevant healthcare software, and knowledge of clinical workflows, data management, and nursing documentation standardsProficiency with Google Suite, Microsoft Word, and Microsoft ExcelBasic knowledge of Medicare and Medicare Advantage guidelinesStrong clinical and assessments skillsDemonstrated skills and experience in staff management, supervision, safety and security practices and procedures within a healthcare setting.Strong supervisory skills with the ability to motivate, train, and evaluate staff; skilled in resolving workplace conflicts and promoting teamworkDemonstrated ability to mentor and educate clinical staff to continue their professional growth and improve performanceAbility to work cross functionally and collaboratively with staff and other operational and clinical leaders to ensure resident and family satisfaction during admission, stay and dischargeExcellent organizational and administrative skills and the ability to handle multiple priorities and maintain accuracy for assessment and documentation in a fast-paced environmentAbility to work as a member of an interdisciplinary team or independently and be self-motivated and goal oriented with shared accountabilityAbility to maintain professional working relationships and communicate professionally with employees, residents, vendor partners, and external healthcare providers.Ability to take initiative to solve problems on behalf of residents and staff members and to exercise sound professional judgment appropriately in urgent situationsAbility to assist licensed personnel as necessaryAbility to regularly and safely lift a minimum of 35 poundsAbility to use a variety of health care equipment such as lifts, thermometer, and blood pressure cuffAbility to stand and walk throughout the shift, as well as to lift and transfer residentsRequirements :
