• Active Vermont license required
• Home health visits with in-home care experience preferred
• EMR : 360
• Population health management experience preferred
• Monday - Friday five 8-hour shifts or four 10-hour shifts
• Min of two years of working experience preferred
• We provide complimentary housing and travel
