Angel Link Member Services Coordinator

The Angel Link Member Services Coordinator supports client engagement and enrollment for Angel Link's Health Home services. This role involves assisting families with Medicaid-related needs, coordinating admissions, maintaining accurate documentation, and supporting a positive experience for clients throughout their journey.

In this role, you will :

• Perform outreach activities to enroll clients in the Health Home program
• Assist clients with Angel Link admissions and ongoing service coordination
• Support families with Medicaid eligibility issues and transitions to adult Medicaid programs / providers
• Maintain accurate records and documentation in compliance with program standards
• Respond to inquiries and concerns via phone and email with professionalism and empathy
• Collaborate with clients, clinicians, and internal teams to ensure a seamless experience
• Contribute to a positive and team-oriented work environment
• Demonstrate reliability by planning and organizing work effectively and meeting productivity expectations
