About The Position

Description :

Provides, coordinates, and directs the provision of home nursing care according to physician orders.

Care is based on agency policies and procedures through the competent application of the nursing process.

Responsible to the Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.

Requires home visits and participation in periodic office meetings.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Average number of patients per day is 5-8.

Contractor will be assigned between 5-10 patients daily.

Each type of visit and activity holds a different point value, with a total of 30 points expected per week.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Proficiency in documentation is essential, and all documentation must be completed within 24 hours of the appointment.

Delayed and incomplete documentation can impact billing and follow-up care.

There are no guaranteed hours with this contract.

Clinicians must hit weekly productivity points of 30, which equates to approximately 40 hours per week.

If receiving insufficient patients, there is an option to float to another clinic to pick up patients.

Traveling may be required up to a certain mileage radius.

Compliance with policies regarding patient assignments is necessary.

All clinicians must be aware this is a home health assignment and responsibilities include driving to patients' homes and attending any required meetings.

Local candidates may apply and be subject to the same requirements.

Prior experience with the required EMR systems is essential for consideration.
