Salary Range : $30160.00 / year

Travel Percentage : Up to 25%

Job Function : The Code Blue Manager will perform a variety of tasks essential to the daily operation of the Code Blue Program.

Essential Functions :

• Develop and enforce a Code Blue Program Plan along with Rules & Regulations
• Interview and screen Code Blue Assistant(s) for hire along with VP / Social Services
• Supervise and provide technical and administrative support to the Code Blue Assistant(s)
• Assure proper Personal Protective Equipment (PPE) supplies are available
• Assure that Code Blue has proper shift coverage
• Communicate with 211 of Code Blue Procedures
• Develop a nightly procedure checklist
• Maintain a working relationship with warming centers and hotels
• Develop a Training Program for Code Blue Assistant(s)
• Develop a screening process for the Code Blue Assistant(s) and Code Blue participants
• Answering incoming calls, recording messages and relaying information
• Develop a referral list of providers for participants who need counseling or other services
• Refer participants to counseling and / or other services as needed
• Meet LOS requirements, spending requirements and deadlines for funded program
• Maintain LOS logs and participants sign in sheets
• Assure participants complete the EmpowOR intake form
• Assure all files are properly documented and prepare case file
• Collect all documents necessary to determine program eligibility
• Enter all participants information into EmpowOR database
• Submit required monthly reports
• Process and complete Purchase Orders
• Assure staff sign-ins and out daily
• Submit bi-weekly timesheets to HR for all Staff
• Attend staff and Community meetings as needed
• Attend monthly meetings with VP / Social Services
• Other as assigned
