Position Summary :

We are seeking a compassionate and detail-oriented Veteran Social Worker to join our team supporting veterans and their families. This role is responsible for developing individualized care plans, coordinating medical and mental health appointments, and assisting with housing, financial, and healthcare applications. The ideal candidate will have experience navigating veteran benefit systems, including VA programs, Medicare, and Medi-Cal, while maintaining a person-centered approach that honors each veterans unique needs and goals.

Key Responsibilities :

• Conduct comprehensive assessments to identify the physical, emotional, and social needs of veterans.
• Develop, implement, and monitor individualized care and service plans.
• Coordinate and follow up on medical, behavioral health, and specialist appointments.
• Assist veterans with completing housing , financial aid , and medical benefit applications.
• Support enrollment and renewal for Medicare, Medi-Cal, and VA benefits programs.
• Advocate for clients by coordinating with physicians, case managers, and community agencies.
• Attend medical or housing appointments when necessary to support the veteran.
• Maintain accurate case notes, billing documentation, and progress reports in compliance with agency and funding requirements.
• Provide crisis intervention and connect clients to emergency or transitional services as needed.
• Monitor outcomes and adjust service plans to ensure optimal results for each veteran.

Qualifications :

• Education : Associate's or Bachelors degree in Social Work, Psychology, Human Services, or a related field (MSW preferred).
• Experience : Minimum 2 years of experience in case management, medical social work, or veteran services.
• Preferred : Knowledge of VA benefit systems, HUD-VASH, SSI / SSDI, and local veteran assistance programs.
• Familiarity with Medicare, Medi-Cal, and community-based healthcare systems.
• Strong documentation, communication, and organizational skills.
• Ability to build trust and rapport with diverse veteran populations.
• Proficiency in Microsoft Office Suite and case management software.
