Location : The position may be based either fully in New York, NY 10027 , or split between offices at Bronx, NY 10455.
Schedule : Monday – Friday from 10 : 00am – 6 : 00pm, or 11 : 00am – 7 : 00pm. CASES is open to discussing part-time hours with applicants!
Workplace Flexibility : Hybrid schedule with one virtual day available upon completion of the 90-day probation period and contingent upon acquiring a sufficient number of telehealth clients.
Salary : $160k- $170k

Job Summary
Client is a New York City agency working at the forefront of the intersection between behavioral health and involvement in the criminal justice system. The Clinic division consists of a continuum of services for justice-involved individuals with serious mental illness; The Nathaniel Clinic is an OMH-licensed mental health outpatient treatment and recovery services clinic that provides outpatient mental health services to adolescents and adults, with a focus on providing care to those involved in the criminal legal system.
The PNP / PMHNP / NPP will work as part of a multidisciplinary team providing coordinated clinical assessments and assertive treatment in all stages of illness. The team nurse practitioner engages the consumer in shared decision making about medication and the next steps in medication treatment. Medication management will be guided by a medication algorithm that provides information about evolving best practices.
Responsibilities
Provide compassionate clinical treatment in accordance with the scope of practice and regulations governing Certified Community Behavioral Health ClinicsConduct up to 12-14 follow ups or 4-6 IPEs per dayConduct psychiatric assessmentsAssist clients, family members, and significant others with concern and empathyWork in collaboration with clients, family, and other collaterals to support recovery goalsRespond to walk-ins and unscheduled client needs and provide expedited services to clients based on risk assessment statusPrescribe medications indicated by diagnosis and lab results, teach clients about side effects, and monitor response to prescribed medicationsComplete buprenorphine training and obtain the waiver to prescribeWork collaboratively with therapists and registered nurses providing specific goals and measurable objectives for inclusion in person / family-centered treatment plansEnsure services are person / family-centered, recovery-oriented, and trauma-informedPromote integration of primary care working collaboratively with the onsite Federally Qualified Health Center (FQHC) medical providers and the clinic’s nurses to ensure the effective delivery of health monitoring servicesPromote and deliver integrated treatment to consumers with co-occurring substance use disordersUtilize best and evidence-based approaches to treatment consistent with client's organizational culture; health / mental health integration, rehabilitation and recovery, and understanding incarceration on treatment engagementProvide culturally competent services that are individualized to client needs and reflect the integration of race, ethnicity, culture, primary language, immigration status, developmental status, criminal justice status, sexuality, age, and genderHelps maintain a safe and clean environment, ensuring proper use and disposal of medical supplies / equipment as required, adheres to infection control protocols, and notifies their supervisor immediately of any potential safety hazard identified in the office / client-care environment.Qualifications
A Nurse Practitioner degree with specialized training or experience in Psychiatry. A current license to practice as a nurse practitioner in the State of New York. Board certified preferable.A current DEA registration for the prescribing of controlled substances. ANCC certification or pending application is required. NYS OPRA status approved or application pending and a Health Commerce Account in place by the onset of employment.Extensive experience working with people with mental illness, serious mental illness and knowledgeable about risk assessment and risk managementTo be eligible for appointment and to maintain employment, you cannot be listed as an excluded individual or entity on any of the Federal and / or State Medicaid and Medicare exclusion lists.
