Key Functions / Responsibilities
Builds relationships with assigned members to understand their concerns, needs, and works toward collaborative goal setting.
Provide educational material to members on programs provided.Provide support and insight that encourages members to attend all care appointments.Assist with scheduling appointments, reminder of appointments, and assisting with obstacles that may prevent attending appointments.Conducts health risk assessments to identify member needs and provides education on the importance of preventive care and support. Increase involvement in personal healthcare decisions, guides members to advocate for more care.May attend community meetings to understand community issues or to build relationships with community members on and as needed basis.Assists members with culturally appropriate Social Determinants of Health (SDOH) needs.Remains up to date on knowledge of community resources relating to various areas of specialty appropriate to the needs of members / families through participation in ongoing trainings, informal learning opportunities, supervisor mentoring, and general professional development opportunities.Engage members in telehealth / videoconferencing to conduct home visits. In person home visits will be conducted only in special cases based on need with advance approval.Adheres to weekly schedule to allow for optimal member outreach and documentation.Attendance at business and department meetings (Stand Up, L10, 1 on 1, trainings, etc.).Conducts additional work assignments corresponding with department needs at the direction of supervisor / manager.Qualifications and Education Requirements
Knowledge of community health resources and social determinants of health
Ability to effectively communicate with a diverse population and establish trustEffective active listening skillsExcellent verbal and written communication skillsBilingual in Spanish (speaking, reading, and writing) a plus, but not requiredExperience using Microsoft Office and data entryAbility to prioritize workflow and manage timeHigh school diploma or GED required. Associate's degree in human services or another correlated field preferred.
Two (2) years of work experience directly related to the duties and responsibilities, or equivalent combination of education, training, and experience preferred .Certified Community Health Worker preferred but not required.Skills
