Social Services Designee
POSITION SUMMARY
The Social Services Designee interacts with the residents, families, visitors, other facility staff, and community agencies during the admission and discharge process. They are responsible for marketing the services we provide and establishing relationships with all referral sources. They clarify a range of legal documents for residents and families as well. As a member of the interdisciplinary care planning team, the Social Service position identifies the psychosocial strengths and needs of each resident, assists in developing an individualized plan of care, and provides medically related social services to help the resident attain or maintain the highest level of functioning possible.
RESPONSIBILITES
Admission and Discharge Coordination : Manages referrals, tours, screenings, approvals, admission paperwork, and discharge planning including referrals for home health services and equipment.
Resident and Family Support : Provides emotional and informational support to residents and families during admission, stay, and discharge; facilitates adjustment to facility life and encourages family involvement.
Psychosocial Assessment and Care Planning : Conducts assessments, maintains psychosocial documentation, participates in care planning, and ensures individual needs are addressed through personalized approaches.
Communication and Liaison Role : Acts as a bridge between residents, families, staff, and community agencies to address concerns, ensure smooth transitions, and support psychosocial needs.
Professionalism and Relationship Management : Maintains courteous, respectful, and professional interactions; follows dress codes and privacy policies; fosters a positive, inclusive workplace culture.
Regulatory Compliance and Continuing Education : Adheres to healthcare regulations, confidentiality standards, and QAPI initiatives; maintains accurate records and participates in ongoing training and policy development.
QUALIFICATIONS
Psychosocial Understanding : Must recognize and effectively address the emotional and psychosocial needs of residents.
Core Competencies : Strong communication, organization, leadership, and management skills are essential.
Preferred Experience : Background in healthcare settings such as skilled nursing facilities or assisted living communities is desirable.
Interpersonal and Adaptive Skills : Must be personable, trustworthy, a strong listener, fluent in English, and able to navigate fast-paced, ambiguous environments with diverse populations.
