We are currently seeking a Licensed Clinical Social Worker (LCSW) in the Los Angeles area who shares our passion for providing dignified, compassionate end-of-life care. This person will be covering the Los Angeles County areas.
Shift Schedule : PRN (Per Diem), Day Shift, Monday (8 : 00am - 5 : 00pm)

Qualifications :
Master's degree in Social Work is requiredDocumented evidence of training in dealing with grief and lossThis role requires a valid driver's license and a good driving record as a condition of employment. Associates must comply with all state and federal driving regulationsAssociates in roles requiring driving are responsible for maintaining a valid and active driver's license. Failure to maintain a valid license may result in reassignment, suspension, or termination in accordance with company policies and applicable lawsReliable transportation and willingness to drive to patient's locationWhat you'll be doing :
Conducts assessments, develops care plans, and documents bereavement activitiesDesigns and distributes materials for families receiving bereavement servicesDevelops educational programs on loss and grief for families and staffEvaluates the bereavement program quarterly and annuallyProvides community consultation and education on hospice servicesMaintains ongoing education through academic studies and workshopsAttends in-services, staff meetings, and case conferencesReports patient condition changes to the RN Case ManagerObserves and addresses safety hazards in patient and organizational environmentsAssists health team in understanding emotional factors related to terminal illnessAssesses suicide risk, mental capacity, and elder / dependent adult abuseAids in advance directives, mortuary arrangements, and financial planningProvides information and referrals to community resources and legal entitiesAdvises and counsels staff, patients, families, or agencies in emergency situations#LI-RB1
