American Traveler is seeking a Physical Therapist with home health experience and Wisconsin state licensure for adult and geriatric patient care.
\n \n Responsibilities\n
Home health setting serving adult and geriatric patientsPrimary site is Wausau with potential to float to Antigo, Rhinelander, Rapids, Merrill, Stanley, and MedfordWork schedule is 4x10 hour shifts with every other Friday off; alternate weeks have another day offRotates one Saturday every 6-8 weeks with a weekday off during that week; no holiday rotationsPatient productivity expectation is an average of 4.5 visits per dayUses NetSmart EMR and a laptop for point-of-service documentation; cell phone used for GPS and communicationMileage reimbursed at government-approved rateEligible for up to 36 hours of low census on-call time; call back minimum is 2 hoursMain responsibilities include evaluation, treatment, and care planning in accordance with PT standardsDocumentation must meet all regulatory and reimbursement requirementsCompliance with flu / COVID vaccination or mandatory masking during flu season is requiredScrub colors are flexible; any color or business casual attire is acceptableTeam-oriented candidates who manage change well and are socially adept are preferredOrientation hours may vary and are not guaranteed during unit closure holidays50 mile radius rule applies until 12 / 9\n \n Requirements\n
