Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities : The primary responsibility of the Child & Family Services Case Manager will be case management services, resource acquisition, crisis intervention, and outreach activities to specified youth and their families. Specific duties include but not limited to :

• providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies;
• delivering services in a variety of locations, including but not limited to the client's home, the school, the Child & Family Services program site, or another community setting, on an as-needed basis;
• providing transportation to and from various activities or appointments as indicated by the treatment plan or as instructed by the QMHP;
• delivering reimbursable hours of service compliant with established standards;
• completing all paperwork associated with the financial, clinical, legal, or regulatory management of the case or the Center's operations in a timely and accurate manner;
• regular and predictable attendance is an essential function of the job.
