Field Care Manager – Behavioral Health (Social Worker)
Location : Wayne or Macomb Counties, MI
Travel : 15–20%, WFH the rest
Residency Requirement : Must reside in Michigan and be able to travel locally
Overview : Judge Healthcare is hiring a Field Care Manager to support Medicare / Medicaid members through in-home and community-based care coordination. This role is ideal for licensed social workers committed to improving health outcomes and promoting member independence.
Key Responsibilities :
- Manage a caseload of Medicare / Medicaid members through in-home, community-based, and telephonic assessments
 - Develop and update individualized care plans in collaboration with members, caregivers, and providers
 - Provide clinical guidance for members with complex medical and behavioral health needs
 - Coordinate services across healthcare, housing, education, and social domains
 - Conduct medical attestations and clinical oversight reviews
 - Ensure care is delivered in the least restrictive setting
 - Lead coordination efforts with the Interdisciplinary Care Team (ICT)
 
Minimum Qualifications :
Bachelor's degree in Social Work (BSW)Active Michigan social work license or limited license with intent to obtain full licensureMinimum 2 years post-degree clinical experience in behavioral healthExperience with adult populations and chronic disease managementStrong knowledge of community health and social service resourcesExcellent communication and interpersonal skillsAbility to work independently and manage time effectivelyProficiency in electronic medical records and Microsoft Office (Word, Outlook, Excel)Valid driver's license, auto insurance (minimum 100 / 300 / 100 coverage), and reliable vehiclePrivate home office setup with high-speed internet (DSL or cable; satellite not permitted)Full-time availability (Monday–Friday, 40 hours / week)Must pass TB screening (patient-facing role)Apply Today!
