Shift : MondayFriday days with rotating weekends and on-call coverage

Job Description :

Sunbelt Staffing is hiring a Home Health RN for a contract assignment in Rock Hill, SC. This role requires prior home health experience with OASIS documentation and proficiency in Homecare Homebase (HCHB). The RN will provide patient-centered care in the home setting while working closely with physicians and the interdisciplinary team.

Key Responsibilities :

Complete OASIS assessments for admissions, discharges, recerts, and resumptions

Deliver skilled nursing care and patient / caregiver education in the home

Chart all visits and care plans in Homecare Homebase (HCHB)

Communicate effectively with physicians and care teams for coordinated treatment

Maintain compliance with agency and Joint Commission standards

Qualifications :

Current South Carolina RN license

At least 2 years of RN experience, with a minimum of 1 year in home health

OASIS and Homecare Homebase (HCHB) experience required

Reliable transportation, valid drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
