Job Summary :

The HAB / CPS Specialist provides direct support services to individuals with intellectual and developmental disabilities (IDD) to enhance their independence, community engagement, and daily living skills. This role focuses on helping individuals build life skills, participate in community activities, and work toward their personal goals in a supportive environment.

Key Responsibilities :

• Provide one-on-one or group support to individuals in community and home settings.
• Assist individuals with developing and maintaining daily living skills, including personal care, communication, and social interactions.
• Facilitate community participation by supporting individuals in attending recreational, volunteer, and social events.
• Encourage independent decision-making and self-advocacy while ensuring a safe and supportive environment.
• Help individuals identify and achieve personal goals that promote independence and community involvement.
• Support individuals with transportation needs to and from activities as necessary.
• Maintain accurate documentation of services provided, progress toward goals, and any incidents.
• Follow all PA Chapter 6100 and 6400 Regulations and adhere to the policies of KZL Agency.
• Collaborate with families, caregivers, and other support staff to ensure a coordinated approach to care.
• Attend training sessions and staff meetings as required.
