Job Description :

Sunbelt Staffing is seeking an experienced Home Health RN for a contract role in Frankfort, KY. This position involves managing a caseload of home health patients, completing OASIS documentation, and entering all visit notes into Homecare Homebase (HCHB). The RN will deliver quality patient care in the field and collaborate with the healthcare team to ensure excellent outcomes.

Key Responsibilities :

Perform admissions, resumptions, recerts, and discharges using OASIS

Provide skilled nursing care and patient / caregiver teaching in the home setting

Maintain timely and accurate charting in Homecare Homebase (HCHB)

Communicate effectively with physicians and interdisciplinary team members

Ensure compliance with agency policies and Joint Commission standards

Qualifications :

Active RN license in Kentucky

At least 2 years of RN experience, with 1 year in home health

Proficiency in OASIS and Homecare Homebase (HCHB) required

Reliable transportation, valid drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
