New Jersey Licensed Case Manager

A company is looking for an RN II-Case Manager (REMOTE).

Key Responsibilities :

Assess patients' clinical needs and ensure appropriate levels of care

Coordinate and implement care plans for members, advocating for their needs

Mentor and train new RN staff and participate in quality improvement initiatives

Required Qualifications, Training, and Education :

High School Diploma / GED required; Bachelor's degree preferred or relevant experience in lieu of degree

Minimum of two (2) years clinical experience required

Minimum of two (2) years' experience with health care payer experience required

Active Unrestricted NJ RN License or active Compact License required

CCM certification preferred for Clinical Advocate roles
