Keystone Health, Idaho’s leading house call provider since 2013, is currently searching for an additional Nurse Practitioner to provide comprehensive medical care to homebound adults who are too ill or medically fragile to travel to traditional clinics.

Responsibilities :

  • Deliver compassionate care directly to elderly patients in their residence, often managing multiple chronic and complex conditions such as stroke, dementia, heart disease, and COPD.
  • Act as the primary clinical provider for housebound individuals, focusing on improving quality of life, functional independence, and symptom management.
  • Collaborate closely with family members, caregivers, and Keystone’s team to ensure continuity and coordination of care.
  • Conduct comprehensive health assessments, physical exams, mental health screenings, and functional evaluations.
  • Diagnose, treat, and manage acute, chronic and degenerative illnesses, and conditions such as dementia and mobility disorders.
  • Provide and oversee individualized care plans in collaboration with patients, families, and caregiving teams.
  • Prescribe medications, order and interpret diagnostic tests, and coordinate specialty referrals as needed.
  • Maintain thorough, accurate clinical documentation and uphold all regulatory and ethical standards required both by Keystone Health and Idaho licensure.

Qualifications :

  • Master’s or Doctoral degree in Nursing from an accredited nurse practitioner program.
  • Current, unrestricted RN and NP licensure in Idaho, with national certification in adult-gerontology or family practice (such as ANCC, AANP, or ACNP credentials).
  • Demonstrated experience in geriatrics, home health, primary care, or chronic disease management (preferably with housebound or medically fragile patient populations).
  • Advanced clinical skills in internal or geriatric medicine
