Your Responsibilities :
- Evaluate and assess patients physical abilities to develop comprehensive, individualized treatment plans.
- Implement and monitor physical therapy interventions to ensure optimal patient outcomes.
- Collaborate with a multidisciplinary team to facilitate coordinated patient care.
- Document all patient interactions and treatment in the electronic medical record (EMR) system.
- Educate patients and their families about rehabilitation plans and home exercise programs.
