Founded in 2012, Urban Street Angels is a San Diego based non-profit dedicated to supporting homeless youth through supportive housing, peer support, mentorship, mental health counseling, and job training initiatives, to develop a more employable workforce and cultivate personal responsibility. Of California's unhoused population, 50% cite they first had an experience with homelessness between the ages of 18 and 25. This critical demographic has the potential to leave the streets behind for good when provided with the support needed to make a change in their lives. Title : Case Manager

Position Summary : The Case Manager establishes and oversees case management services for youth served by Urban Street Angels. The Case Manager provides case management services within a designated program on an ongoing basis but also helps to develop programs. The Case Manager will report directly to the Program Manager, working closely with other areas and departments within the organization, including peer support, housing & outreach, medical services, as well as subcontractors and outside service providers.
Responsibilities :
- Conducts participant intake, gathers required identification and documentation, and sets up both electronic and hard copy files.
- Develops and maintains individualized case and housing stability plans based on participant needs.
- Maintains regular face-to-face engagement with youth to support progress in areas such as housing, education, employment, and life skills.
- Coordinates services with internal team members and external providers to ensure effective support and resource linkage.
- Completes monthly documentation, including productivity verification, case notes, financial tracking, and referrals.
- Supports participants in building positive adult relationships and developing self-regulation, self-efficacy, and independent living skills.
- Manages all aspects of program discharge, including coordination of move-out logistics, transfer of records, and exit data entry.
- Ensures timely and accurate data entry into Homeless Management Information System (HMIS) and Efforts to Outcomes (ETO).
- Maintains thorough documentation of all interactions, services, and participant progress, meeting required timelines and quality standards.
- Team-Oriented Responsibilities : Must be adaptable and willing to take on additional tasks or project as assigned, contributing to a collaborative an responsive team environment.
Qualifications :
Bachelors degree required - preferably in social services, social sciences, psychology or related fieldShared life experience and / or relevant case management experience focused on at-risk young adults (ages 18-24)Minimum of one year experience working in a social service setting, serving individuals with severe mental illness or otherwise show demonstrated ability to relate to the needs and concernsStrong interpersonal and communication skills both written and verbalDemonstrated knowledge of trauma-informed care, harm-reduction models, advocacy and support techniques for victims / survivors of violence, individuals with mental illness and / or substance abuse issuesAbility to work independently and as part of a team, manage time effectively, and prioritize tasksCultural sensitivity and experience working with diverse populationsProficiency with Google Workspace, preferredExperience electronic case management systems, including note taking and documentationAbility to advocate effectively and maintain professional boundariesAdditional Requirements :
