This will be a full time or part time day shift- flexible hours / schedule

Full Time needs are all over the territory

Part time needs focus mainly on Centerville Kettering and Beavercreek area.

Pay Range : 42.50-48.80 (Base) pay per visit point system / increases based on visit type

Position Summary :

Responsible for evaluation plan of care treatment re-evaluation discharge documentation and modification of all client programs. Administers physical therapy in compliance with Federal State local and Bethany Village Home Health Care standards policies and guidelines.

Job duties :

• Administers physical therapy in compliance with Federal State local and Bethany Village Home Health Care standards policies and guidelines ensuring quality care and service to clients.
• Interacts with health professionals clients families and support groups to ensure maximum rehabilitation while serving as a client advocate at all times.
• Evaluates clients by administering muscle neurological gait and functional tests.
• Establishes treatment goals based on the evaluation of the client in conjunction with the clients needs.
• Develops treatment programs and provides ongoing assessments and modifications as changes in the clients condition occur.
• Prepares transmits and maintains timely and accurate records to include the initial evaluation progress notes discharge summary and attendance records in accordance with agency policy and that support charges for services provided.
• Promptly reports incidents accidents changes of condition abuse concerns to the assigned staff nurse or the Director of Bethany Village Home Health Care .
• Complies with OSHA standards and ensures the safety of the client in their care at all times.
