Licensed Vocational Nurse (LVN) – Six-Bed Congregate Living Health Facility

We are seeking a compassionate and dependable LVN to join our care team at our six-bed Congregate Living Health Facility in Victorville, CA. This position offers the opportunity to provide high-quality, individualized care in a supportive and structured home-like environment.

Responsibilities :

Provide daily nursing care and monitoring for residents with chronic or complex medical needs.

Administer prescribed medications and treatments accurately and in accordance with physician orders.

Monitor vital signs, report changes in condition, and document all care in compliance with Title 22 and facility policies.

Collaborate with the interdisciplinary team (RN, physicians, administrators, direct support staff) to ensure continuity of care.

Maintain proper infection control practices, including sterilization of medical tools and maintaining a safe clinical environment.

Support residents with health education and ensure families are kept informed of changes in care plans.

Uphold dignity, respect, and compassion in all resident interactions.

Qualifications :

Current California LVN license in good standing (required).

Previous experience in skilled nursing, congregate living, long-term care, or a similar setting preferred.

Strong clinical assessment skills and familiarity with medical tools and equipment.

Ability to build positive rapport with residents, families, and staff.

Excellent written and verbal communication skills.

Compassionate, patient, and professional demeanor.
