In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services.\n\nEssential Responsibilities\n
Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals\n
Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families\n
Educate patients and families on what hospice care includes, how it works, and why it matters—providing clarity, reassurance, and compassion\n
Complete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met\n
Serve as a liaison between referral sources and our hospice care team—ensuring seamless handoffs, clear communication, and rapid response to needs\n
Make daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians\n
Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services\n\nCompensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity. Licensed Nurse with three to five years of hospice experience required.\n
BSN with five or more years of hospice experience preferred.\n\nExperience\n
Minimum of three years of hospice experience required.\n
Prior experience as a discharge planner, case manager, or educator preferred.\n\nLicenses and Certifications\n
Current RN license to practice nursing in the state of employment.\n
CPR certification.\n
Valid driver's license and automobile insurance.\n\nSkills and Knowledge\n
Strong knowledge of hospice care and the unique needs of patients and families.\n
Familiarity with community resources and referral networks.\n
Excellent patient assessment and documentation skills.\n
Ability to supervise staff, coordinate services, and manage programs effectively.\n
Strong communication and interpersonal skills with sensitivity to diverse backgrounds.\n\nPersonal Qualities\n
Compassion and sensitivity to the needs of terminally ill patients and their families.\n
Flexibility to manage a variety of tasks and shifting priorities.\n
