Position Purpose
Provides direct nursing and wellness care to the Assisted Living Community (ALC) residents aka Villagers and supervises the day-to-day clinical activities performed by the licensed practical / vocational nurse and Certified Nursing Assistants (CNAs) and Certified Medication Aide (CMAs) in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures. The LPN Nurse Manager position will be the liaison between the Villager, Villager family, designated pharmacy and Villager medical providers. The LPN Nurse Manager is an integral part of the Assisted Living Community to ensure the Villagers receive holistic care.
Required Qualifications
2 years of management experience in long-term care (Assisted Living) required.
A Nursing degree from an accredited college or university with at least 4 years of nursing experience.
Current unrestricted license as a Licensed Practical Nurse (LPN) in Georgia and active CPR certification.
Two (2) years’ experience working with adults with developmental disabilities and med / surg nursing preferred.
Have proficient computer skills with working knowledge of programs such as Word, Excel, Outlook, etc.
Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements.
Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
Must undergo and pass a drug screening and screening for Tuberculosis.
Major Duties and Responsibilities
Directs the daily activities of the Assisted Living Community (ALC) personnel including Certified Medication Aide (CMAs) and Certified Nursing Assistants (CNAs) in accordance with federal, state, and local regulations and facility policies and procedures.
Participates in Villager admissions, discharges, and transfers in collaboration with the Admissions Team.
Assesses and monitors changes in Villager health status, notifies physicians and family / representatives accordingly, and documents actions taken.
Prepares, administers, and monitors the effects of medications as prescribed, including observation and documentation of any adverse reactions.
Completes the full process of medication management including order processing, re-ordering, delivery coordination, verification, and check-in of pharmacy medications.
Conducts weekly audits of medication carts to ensure compliance with regulatory standards and best practices.
Provides diabetes device management, including maintenance, monitoring, and documentation of insulin administration or glucose readings.
Conducts daily Villager rounds to monitor health status, address concerns, and support well-being.
Reviews and follows up on the 24-hour report daily to ensure appropriate interventions, care updates, and documentation.
Schedules follow-up appointments and coordinates transportation as needed for Villager healthcare visits.
Serves as back-up support for the Resident Appointment Scheduler and Certified Medication Aide Wellness Coordinator, assisting with appointment logistics and staff coverage.
Collaborates with interdisciplinary team members to ensure comprehensive care planning and coordination.
Participates in management team meetings to discuss census updates, resident care changes, and departmental concerns.
Evaluates nursing and clinical staff performance and implements disciplinary actions as needed per organizational policies.
Serves as the primary liaison between Annandale’s designated physician group and pharmacy, ensuring seamless communication and coordination.
Serves as an emergency point of contact, nurse on call, during nights and weekends for acute medical needs.
Ensures adequate inventory of medications, medical supplies, and equipment; communicates needs to appropriate departments.
Initiates, reviews, and updates individualized Villager care plans as necessary.
Promotes teamwork, mutual respect, and effective communication among staff.
Actively participates in Quality Assurance and Performance Improvement (QAPI) and Continuous Quality Improvement (CQI) activities.
Participates in external inspection surveys and supports survey readiness and compliance.
Monitors and ensures adherence to infection control practices; identifies and addresses areas of concern.
Reports grievances, incidents, and unusual events to the Assisted Living Administrator, Senior Nursing Home Administrator, and Compliance Director as required, and participates in investigations.
Demonstrates the ability to work independently with minimal supervision, exercising sound judgment and decision-making.
Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care of the Villagers.
