o   EPIC experience is required

o   Skilled in placement for SNF / IPR / LTAC / Hospice / Home Health, DME, Behavioral Health needs / DSS (peds), Difficult complex placement, will work routinely with homeless population

o   Care management skills : assessment, care coordination and discharge planning
