Location : Kings County Hospital, 451 Clarkson Avenue, Brooklyn, NY 11203
Shift : Days – 5x8-Hour (8 : 00 AM – 4 : 30 PM)
Job Description :
The Wound Care RN provides specialized nursing care in the areas of wound, ostomy, and continence management. This role involves patient assessment, care planning, treatment administration, staff education, and collaboration with the healthcare team to ensure high-quality patient outcomes.
Key Responsibilities :
Conduct comprehensive health assessments focused on wound, ostomy, and continence care.Develop and implement individualized plans of care, recommending appropriate medications and treatments.Assess and document patient responses to wound / ostomy / continence treatments and escalate issues as necessary.Collaborate with interdisciplinary teams and refer patients to additional disciplines when indicated.Provide patient, family, and staff education on wound prevention, ostomy care, and continence management.Initiate preventive measures for patients at risk for pressure injuries.Participate in unit and hospital-wide Quality Improvement (QI) activities (e.g., incidence / prevalence studies).Assist in the selection of wound care supplies, technology, and equipment (including specialty beds and negative pressure devices).Maintain cultural competency in care delivery and uphold patient confidentiality.Support workforce development through orientation, in-service training, and continuing education programs.Requirements
Experience : Minimum 2 years of Med / Surg experience required; prior wound and colostomy experience strongly preferred.Education : Associate’s Degree in Nursing (ASN / ADN) from an accredited nursing program required.Licensure / Certifications :Active New York State RN License (Primary Source Verification through NYSED required).
AHA BLS certification.At least one current certification in wound / ostomy / continence care :Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB
