Your Role as a Hospice LPN

• Deliver hands-on nursing care to hospice patients in their homes.
• Administer medications, conduct wound care, IV therapy, and other essential treatments.
• Educate and support patients and families about disease processes, symptom management, and end-of-life care.
• Collaborate with the RN Case Manager and the interdisciplinary team to ensure cohesive and compassionate care.
• Accurately document clinical visits and interventions timely.
