\nPT- home health needed asap\nPlans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.\nBachelor's Degree or equivalent (some certifications will require Bachelor?s) : Required\nMaster's Degree : Preferred\nPhysical Therapy licensure in all applicable states of practice : Required\nCardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification : Required\nValid Driver's License (DL) and must be at least 21 yrs of age or older : Required\n
